is current provider. Employee is eligible after
90 days of employment. Open enrollment is within
first 90 days of employment. Otherwise you must
wait until next open enrollment date.
$20,000 term insurance at no cost to the
employee.
Conducted January 1 each year.
Company matches up to 3% of total annual
compensation. Employee may contribute up to
